About this role

Engineers who thrive on diving into complex, messy datasets and transforming raw information into tangible clinical outcomes will find a natural home at Iterative Health. This Cambridge and New York-based healthcare technology company builds a performance-driven network that accelerates clinical research through advanced artificial intelligence and deep trial expertise. The Applied AI Engineer will work hands-on with a unique dataset connecting electronic medical records, endoscopic video, and trial data across more than eighty sites to construct production-ready machine learning models, LLM-based systems, and agent pipelines.

This role exists at the formative stage of a small, fast-moving engineering team where the foundational predictive capabilities are actively taking shape. Team members collaborate closely with product and engineering partners to define the complete path from exploratory data analysis to shipped features that improve patient outcomes. Ideal candidates bring substantial hands-on experience building systems on real-world data, coupled with the judgment to separate practical AI applications from industry hype.

What you will do

  • Analyze clinical, video, and operational trial data to identify meaningful patterns and drive efficiency across clinical trial systems.
  • Design, build, and deploy machine learning models, LLM-powered applications, and agent architectures from initial prototype to production.
  • Evaluate emerging AI and machine learning capabilities to determine their practical application to company datasets and specific problems.
  • Partner with product and engineering teams to translate analytical findings into shipped operational features.
  • Establish the pipeline from raw data to product by defining modeling approaches, evaluation strategies, and production readiness criteria.
  • Identify unique predictive advantages within proprietary healthcare data and decide where to build or partner.
  • Contribute to the engineering culture of a growing team by shaping technical decision-making and establishing practical rigor.

What you need

  • Bring five or more years of professional experience in applied AI, machine learning, software engineering, or data science with a strong track record of shipping production systems.
  • Possess hands-on familiarity with healthcare data, clinical research, or life sciences.
  • Demonstrate strong statistical foundations and the ability to distinguish meaningful results from noise.
  • Show fluency in Python and SQL with the capability to work across the entire data stack from exploratory analysis to production code.
  • Exhibit experience building production applications utilizing LLM features such as prompting, evaluation, RAG, and agent pipelines.
  • Maintain the capability to communicate complex technical findings clearly to both technical and non-technical stakeholders.

Nice to have

  • Experience working at growth-stage startups
  • Experience with LLM application frameworks including RAG, agent orchestration, and evaluation harnesses
  • Familiarity with clinical trial operations, disease classification, or patient identification problems
  • Experience building or defining the roadmap for an early-stage AI function

Benefits

  • Hybrid work environment with in-office collaboration two days per week in NYC, Southlake, or Boston
  • Comprehensive medical, dental, and vision coverage with up to 80% of premiums covered
  • Mental health and wellness support through Spring Health alongside HSA and FSA options
  • Unlimited PTO, 12 company holidays, and a company-wide shutdown between Christmas and New Year
  • 401(k) program with a company match of up to 3% up to $3,000 annually
  • Annual wellness and professional development stipend
